INFORMATION

GENERAL

Highland Care Center provides skilled nursing care and rehabilitation.  We offer a variety of services, including physical therapy, occupational and speech therapy.

ADMISSIONS

The Admission process takes approximately one hour to complete.  Information must be gathered, recorded and documents signed.  Often it is possible to do this prior to admission into the facility.  The admission coordinator will schedule this with you.  We are always happy to send someone to a location convenient to you to assist in completing the necessary paperwork prior to admission.

Following is a list of items we will need to complete the admission process:

• Social Security Card
• Medicare Card
• Other insurance card
• Applicant’s full legal name
• Applicant’s date of birth
• Responsible party’s name, address and phone numbers
• Emergency contact’s name and phone numbers
• Copy of Living Will and/or Power of Attorney

PHYSICIANS

Each resident must have a physician who will follow their care during their stay at the care center.  Some physicians come to the care center to see the resident.  Many require the resident go to their office, and some will ask that our Medical Director (house physician) perform follow-up care.  We provide transportation to the physician’s office at no additional charge.

MEDICARE

DAY 1 – DAY 20
If eligible for covered services, MEDICARE pays 100%

DAY 21- DAY 100
If still eligible for covered services, Medicare pays room, nursing and therapy costs minus $137.50 per day co-pay.  This can be an out of pocket expense.  If you have a Medicare supplemental insurance, this cost may be covered.  We are happy to check your coverage for you.

DAY 101+
Medicare pays nothing.  If services are still required, room and board is paid out of pocket.  Medicare Part B covers 80% of therapy services, however.

INSURANCE

We are contracted with many different Insurance Companies and we are adding more all the time.  We will check your insurance for you and let you know, prior to admission, what will be covered.
